Please bring both the COVID-19 Immunization Consent Form with an Adult or Minor Form (whichever is applicable) filled out, as the paperwork needs to be completed prior to receiving your vaccine.

Please bring your insurance card and, if you are 65 or older, bring your Medicare A/B card.
